	Replacement Certificate of Registry
	A replacement Certificate of Registry can be issued where the original certificate is lost, mislaid, or destroyed. Required Documents: Application for New Certificate of Registry/Certificate of Registration form SR26. Fee of $169 Full details of the circumstances of the loss of the original Certificate must be given in the application form.

If the original Certificate is found after a replacement has been issued, the original must be returned to the Registrar for cancellation.
